Job Description

Mission-driven social service agency is seeking a hardworking, self-motivated Service Provider to provide support services to individuals with developmental disabilities. This position is primarily responsible for assisting individuals living in their own homes to become valued members of their community through skill building and development of natural supports.

Summary: Support may occur in a variety of settings and may include providing personal care, companionship, assistance with activities of daily living, assistance with communication and self-expression, and ultimately supporting children and adults with developmental disabilities in a variety of home and community settings.

Flexible schedule needed. 20-30 hours/weekly.

Essential Duties and Responsibilities

  • Provide direct assistance and support to individuals with their personal care needs.
  • Assist with grocery shopping and meal preparation.
  • Coordinate recreational and educational activities.
  • Assist with light housekeeping (e.g. laundry, dishes, vacuuming) as needed.
  • Monitor and address medical needs of individuals served.
  • Coordinate with day services, employment, educational activities, counseling services, volunteer activities, community involvement and/or other services.

Job Qualifications

  • Experience working with children/adults with disabilities preferred.
  • Valid driver's license
  • Proof of auto insurance and vehicle registration required.
  • Must have reliable transportation.
  • Ability to communicate clearly in a variety of situations is essential.
  • 18 years of age or older
